The report found that over the last four years, Frontier has been understaffed and underpaid, and has faced financial and legal problems.
"Folks have been left with significant problems and concerns about Frontier's performance with respect to service and communications," the report said. "The company continues to be understaffed and underpaid, and has been subject to significant delays and expenses since its inception as a telecommunications company in 1993."
The report said Frontier has been able to meet its expenses because of its strong relationship with its customer base, including some of the "most valuable customers" whose service has been affected by outages.
Folks in Minnesota often get high rates for their service, and many others have to wait a couple of weeks to receive their phone calls.
In one instance, Frontier customers who used the customer's name changed their billing records, including the name of the "John Doe" number for the phone line they called from, the report said.
The report also found that Frontier has repeatedly failed to notify customers about problems with its data center, including data and security breaches.
"A common feature of Frontier's data center is failed communications between customers who cannot access the company's network via a wired Internet connection but have access to it through a wired Internet connection," the report said.
Folks also failed to provide customers with any information about the company's customer service and data center, said the report, including customer service reports.
Folks told the Minnesota Commission that it "has been in touch with many of the Frontier customers who have reported issues with their wireless service in Minnesota or had their wireless service disconnected from their networks by the company."
Folks also reported to the commission that the company has "no plans to replace or reform the service."
The Minnesota FCC asked Frontier to address this issue and to address problems with Sprint's data center, the report said.
Folks also said that Frontier has been unable to provide customers any information about how to update their wireless network.
